Dischargeability: from a Client of 10 years

The bi-monthly care team meeting is a great example of this, as it is something we requested and it is an integral part of the success of Rosa’s program of support. This has become increasingly important as Rosa’s cognition and mobility have declined and she now requires 24/7 support. ”

Rosa is a woman in her 90’s and has been a client with AAC for 10 years. Rosa suffered an ABI following a road trauma incident and experiences significant issues with cognition, mobility and the ability to live independently, requiring full assistance with activities of daily living.

Prior to her injury, Rosa was a very adventurous, outgoing and active 80-year-old, travelling extensively and enjoying activities such as bush walking and snorkelling. Rosa has a very supportive family who are highly involved in the coordination of her care and support.

How long were they hospitalised for?

Rosa was hospitalised for 3 months in an acute hospital and was then admitted to a respite unit for rehabilitation. Rosa’s family felt at this stage, that the hospital seemed to be under significant pressure to discharge Rosa and they felt hurried and unsure about the future, especially as Rosa’s home required modification to allow her to live and be supported safely.

What were some of the main barriers to getting home?

Rosa came under the NSW Lifetime Care and Support scheme and the family found the paperwork and ‘red tape’ to be a barrier initially.

They were however, supported by a Case Manager at LTCS to complete all the documentation and were also advised about various support agencies to access.

How did AAC support your transition home, and what worked particularly well?

Initially, Rosa’s family chose another provider but found their service to be unsatisfactory and not tailored to Rosa’s needs. Communication was a major issue.

The family elected to change providers and contacted AAC. A senior representative from AAC (Danielle) met with the family within 24 hours of receipt of the enquiry to discuss Rosa’s support needs and how services could be improved so that the family felt comfortable, informed and confident that Rosa’s supports would be managed in a professional and respectful manner.

For the family, engaging AAC was a turning point, as they felt reassured that the support provided to Rosa was comprehensive, addressing not only the physical needs but also the emotional and logistical aspects. They also felt confident that any issues raised by them would be addressed and that their concerns would be listened to.

They were also very pleased to know that AAC would facilitate a bi-monthly care team meeting – a forum for all members of Rosa’s AAC care team, LTCS, GP and the family to discuss all aspects of Rosa’s support and any issues that may have arisen such as changes in Rosa’s condition or ability to express her needs, changes in medication or behaviour etc.

How long did it take to return home, once AAC was engaged?

Rosa was already at home when AAC was engaged and the switch from the previous provider was handled swiftly and professionally by AAC. Rosa’s family are extremely happy with the AAC support team.
