Come along to hear about AAC’s highly successful Dischargeability program, a seamless rapid hospital discharge initiative that supports a coordinated approach to expedite discharge for patients with multiple and/or complex disabilities deemed medically fit to be released from hospital.

  • Dischargeability is a free service provided by AAC
  • AAC understands the complexities of discharging patients with multiple and complex disabilities from hospital – we’re here to help and will work alongside the multidisciplinary acute care team to facilitate smooth and timely discharge that supports a goal-oriented approach for patients to be discharged home or to interim accommodation
  • Learn about our 12-step process to seamless transition and how we support hospital teams to navigate complex funding models, source appropriate and accessible accommodation, develop goal-oriented transition plans and resolve common barriers to discharge
  • We support clients right across NSW – metro, regional and rural locations
  • Hear from our highly experienced and dedicated Dischargeability Managers.
  • Hear from clients we have successfully transitioned home from hospital after long and complicated hospital stays
  • Light supper provided

The closest parking is Wilson parking on 80 George St, with entry from Andrew Nash lane, or Secure parking located on 75 George St.

The closest train station to the City Group rooms west is Parramatta station. Or, if you are travelling by ferry, the River Cat Wharf.
